ABSTRACT: Pavement cracking on Highway 6 was noted at the beginning of October 2014. The site is located approximately 13 km south of Southey, Saskatchewan. The fill section is approximately 400 m long, and the embankment was up to 30 m high with a 2H: 1V side slope, east of the highway. The failure was located within the highest fill embankment section and was indicated by arc-shaped cracking on pavement surface, vertical displacement of the road surface, and deflection of the guardrail. The failure impacted a section of the highway approximately 70 m long. A detailed site investigation and slope stability study was conducted to develop a response plan. The analyses were conducted using both deterministic and probabilistic slope stability analyses. The analyses were performed for various potential slope failure mechanisms at representative cross-sections. This paper presents the results of geohazard risk assessment, terrain analysis, site investigation, instrumentation monitoring, numerical slope stability analysis and evaluation of remediation options.
